International TB Surveillance Data Collection

International Case Search Page - retrieves existing TB cases.	International TB Case Form Page 1- includes demographic information and basic disease information.	International TB Case Form Page 2 - includes diagnostic information.	International TB Case Form Page 3 - includes risk factor and treatment information. This page is the end of the initial case data collection form.
International Follow Up 1 Form - includes laboratory results that come back while the patient is in treatment. This form is completed midway through treatment and is a one-page form.	International Follow Up Final 1 Form Page 1 - completed at the end of the patient's TB treatment. This page includes outcome treatment and disease information.	Follow Up Final 1 Form Page 2 - includes outcome information at the completion of treatment.

CDC RVCT Follow Up 1 Form - includes laboratory results conducted part way through patient treatment. Form is completed midway through TB treatment.	CDC RVCT Follow Up 2 Form Page 1 - includes completion of treatment and outcome information.	CDC RVCT Follow Up 2 Form Page 2 - includes completion of treatment information on Directly Observed Therapy and Final Drug Susceptibility.
